  • Apply for free service credit to cover your military leave from an IPERS-covered employer. For additional information on military benefits, visit the military benefits page.

Application for IPERS Retirement Benefits

  • Regular members may apply for retirement online via My Account. Visit the My Account page for details on logging in. Protection Occupations and Sheriffs/Deputy Sheriffs members must apply for retirement using a paper application. Please contact an IPERS retirement counselor to request an application.

Application for military leave contributions

  • Complete this application to make up your IPERS contributions which were unpaid while you were on military leave from an IPERS-covered employer.       

Application for service purchase

  • Submit this form to purchase additional service credit. The more years of service you have, the higher your monthly benefit will be at retirement.

Beneficiary designation

  • Use this form to designate or change your IPERS beneficiary. You can also update your beneficiary designation in My Account
    NOTE: If you are married and wish to designate someone other than your spouse, you must print, sign and return the form to IPERS.   

Election for termination of IPERS coverage

  • For some public employees who can opt out of IPERS within the first 60 days of employment.

Name or address change

  • You can update your contact information directly in My Account or complete this form and return to IPERS.

Release of information authorization

  • Complete t form to allow IPERS to temporarily release and discuss specific information from your account with a third party.

Request for IPERS Benefit Estimate

  • You can always view a benefit estimate in My Account. You may also complete this form to have IPERS send you a copy of your benefit estimate.
